Overview:

Parkview Health is currently seeking an experienced Inpatient RN Case Manager to join our collaborative Care Team. As an RN Case Manager, you will play a vital role in coordinating patient care, driving stay progression, and facilitating safe, efficient discharges. This position is essential to optimizing patient outcomes and transitions of care.

Key Responsibilities:

• Conduct comprehensive patient assessments to guide discharge planning and determine patient status.
• Collaborate closely with the interdisciplinary Care Team to ensure seamless communication and care progression.
• Drive the progression of patient stay by anticipating needs and proactively managing discharge plans.
• Expedite transitions to the next level of care through detailed, patient-centered planning.
• Maintain accurate and timely documentation using Epic.

Technology & Equipment Used:

• Epic for all documentation
• Mobile device/cell phone for internal communication

Qualifications:

Required Skills & Experience:

• Minimum of 2 years' experience as an Inpatient RN Case Manager
• Proven ability to work independently and manage time efficiently
• Strong collaboration skills within a multi-disciplinary team
• Ability to recognize and initiate work without being prompted
• Proficient in conducting patient assessments and discharge planning
• Strong written and electronic documentation skills

Licensure & Certifications:

• Active RN license in Indiana or a compact state license
• Case Management certification preferred

Schedule & Work Environment:

• Standard 8-hour shifts (exact shift to be determined by department need)
• Weekend rotation is required
• Call schedule may be required and will be specified per position
• While you will be assigned primarily to one unit, occasional floating to other units may be needed. Floating will only involve Inpatient Case Management work, which is consistent across units.

Unit Information:

• Units range from 20 to 36 beds
• Units typically have 1–2 RN Case Managers based on size
• You will never be the only RN Case Manager on duty
• Nurse-to-patient ratio varies by unit (nursing only)

Dress Code:

Choose one of the following options:

• Business casual with a white lab coat (no jeans, closed-toe shoes)
• Eggplant-colored scrubs (top and bottom)

Training & Orientation:

• Parkview General Orientation: 6 hours
• Epic Training: 4 hours
• Peer Training: 8–16 hours of role-specific shadowing
• Limited assignment with oversight: 8–16 hours
